"Heaven Knows I'm Miserable Now" is about the frustrating experience of being in an unhappy, disconnected relationship. The singer reflects on his past life when he was "happy in the haze of a drunken hour", and then contrasts it with his miserable present reality. The song criticizes the other person in the relationship, recognizing they don't care if the singer "lives or dies", and bemoaning their emotional detachment. The singer questions why he even bothers to put effort into the relationship, when it's clear the other person doesn't care. In the end, he recognizes the need to "naturally" flee this dead-end relationship.
